Pediatrician (3410-18 Broadway)

City Prime Health is a patient-centered, team-based multi-specialty practice that has a full-time opportunity available for a Pediatrician in Harlem, NY. Our mission is to provide the most professional, courteous, and highest quality health care, with dignity, to those we serve, especially the underserved population.

We are seeking a Full-Time Pediatrician to provide quality, competent, safe, and compassionate care based on established standards in patient-focused preventative and treatment care. The Pediatrician will work closely with a team of professionals to provide culturally sensitive, high-quality, comprehensive health services to the community to achieve expected outcomes and comply with our Mission.

Job Responsibilities:

• Conduct age-appropriate assessments of patients, including history taking and physical examination in accordance with current standards of professional practice, regulatory requirements, and agency policies and procedures.
• Perform assessment/data collection in an ongoing systematic manner, focusing on physiological, psychosocial, cultural, spiritual, and cognitive status, patient/family education, family involvement, and patient advocacy
• Consult with other clinicians as dictated by patient need and assessment findings
• Participate with physicians and other clinicians in the department to ensure effective patient care consistent with quality indicators for patients cared for in the designated service
• Review medical records for follow up or ordering of diagnostic studies under established protocols
• Provides primary medical care to patients and manages acute and chronic conditions.
• Assesses patients' conditions and age-specific medical needs; sets goals and prescribes appropriate clinical interventions to meet these goals.
• Prescribes appropriate laboratory and diagnostic procedures related to patients' health status and facilitates appropriate treatment intervention predicated on the results of these procedures.
• Provides comprehensive medical services for members of the family, regardless of age or sex, on a continuing basis: Examines patients, using medical instruments and equipment.
• Participates in quality assessment and improvement activities and achieves productivity goals. Examination of patients, formulation differential diagnostic plans, defines and orders required diagnostic testing.
• Interpretation of examination findings and test results, and implementation of treatment plans.
• Determines the need for consultation and assists in medical care and treatment provided at the direction of other specialists.
• Answering patient telephone consults with the assistance of clinic staff.
• Orders or executes various tests, analyses, and diagnostic images to provide information on the patient's condition.
• Maintains accurate and detailed medical records about each patient, including observations during examinations (via EMR).
• Administers or prescribes treatments and medications.
• Promotes health by advising patients concerning diet, hygiene, and methods for the prevention of disease.
• Writes orders to inoculates and vaccinate patients to immunize patients from communicable diseases.
• Refers patients to a medical specialist for consultant services when necessary for the patient's well-being.
• Works continually to assure deliverance of quality clinical care in a safe, efficient, competent, and effective manner.
• Prepares records and reports as required in support of services rendered in accordance with established procedures.
• Comply with all laws applicable to and practice within the scope of Family Practice, including HIPPA.


Education/Experience Required:

• NYS license as Pediatrician
• Valid NPI & DEA

Other Related Skills/Experience:

• Must have work experience in community-based health care
• Must have an understanding of EMRs
• Ability to demonstrate teamwork and effective interpersonal relations skills.
• Excellent written/oral communication and computer skills
• Ability to foster and maintain a positive working relationship in a multidisciplinary team and clients
• Spanish speaking is a plus!
